The End of Life Care Team can provide details of training, events etc of the County and these will be posted here.

 
Advance Care Planning (ACP) is an important part of End of Life Care and is a crucial part of the End of Life Care Strategy (2008). ACP is used to support an individual’s wishes and preferences related to their care needs, if for any reason, they are unable to make those wishes known for them.

NHS Gloucestershire is promoting the roll out of Advance Care Planning within Care Homes with the support of the National End of Life Care Programme by the use of four county wide half day work shops.

We are inviting all Care Homes to attend one of these free workshops (manager and end of life care link worker) in order to empower care homes to confidently introduce the ACP within their own care setting.

The ACP workshops will be held on at the following locations and will be from 12.30 – 4.30, lunch and beverages provided.

·         Cotswold Care Hospice – 7th March 2012

·         Mill House Nursing Home, Chipping Campden – 14th March 2012

·         Gloucester College, Oxstalls Campus – 21st March 2012

·         Great Oaks Hospice  - 11th April 2012

Following Maggie Martin's attendance at The Learning Exchange on 6th September 2011, we are delighted to be able to provide hard copies of the Liverpool Care Pathway (LCP) for Care Homes.  Maggie will remain in post for a further year and is keen to work with Homes facilitating End of Life Care Education and Training in partnership with the Care Home Support Team.

The GCPA will hold a stock of 200 copies of the LCP, these will be available to members at cost price.  Training is also available from the Care Home Support Team.

It is suggested that the above workbook is an ideal start for all existing and new staff to introduce them to End of Life Care.  The workbook takes approximately 2 - 3 hours to complete and the learner will need a mentor.

This questionnaire can help assess an individual or Team's confidence in delivering End of Life Care.  If completed please send to maggie.martin@glos.nhs.uk and she will help provide a tailored action plan for Education and Developments needs.

Maggie explained that these items are not compulsory but we strongly support this guidance for our members as a way to help to provide good End of Life Care.  Maggie is keen to hear from anyone using these tools and especially if you need further assistance.

End of Life Care for All (e-ELCA) is an e-learning project, commissioned by the Department of Health and delivered by e-Learning for Healthcare (e-LfH) in partnership with the Association for Palliative Medicine of Great Britain and Ireland to support the implementation of the Department of Health’s national End of Life Care Strategy (July 2008).

e-ELCA aims to enhance the training and education of all those involved in delivering end of life care to people, so that well-informed high quality care can be delivered by confident and competent staff and volunteers, across health and social care, wherever the person happens to be.

Around 130 highly interactive sessions of e-learning are available in four specific areas:

  • Advance Care Planning
  • Assessment
  • Communications Skills
  • Symptom Management

A fifth area is devoted to presenting the learner with a variety of clinical situations to integrate and consolidate learning about end of life care.

e-ELCA is currently being rolled out to all health and social care professionals. A small number of sessions are also freely available to volunteers and clerical and administrative staff on an open access website. For further details see ‘Access the e-learning’.

Managing Memory 2gether

Contrary to popular belief memory problems are not a symptom of old age. Stress, anxiety and depression can all affect memory.   If you have concerns about your own or someone else’s memory, it is important to seek expert help and advice as early as possible.

In Gloucestershire, the 2gether NHS Foundation Trust is at the forefront of meeting the needs of people with memory problems and their carers through its Managing Memory 2gether service.

Managing Memory 2gether includes a Memory Assessment Service offering early diagnosis, treatment and an information and education service that also offers free education sessions.

If you are worried about your memory it is important to visit your GP first.  However, if you feel unable to talk to your GP about your memory problems, or those of a family member, you can contact Managing Memory 2gether direct to discuss concerns. If appropriate, an appointment will then be arranged with the Memory Assessment Team.

Managing Memory 2gether’s information and education sessions offer people with memory problems and carers the opportunity to meet others in a friendly environment where they can meet other people who have similar concerns. The short (2 hour) sessions are held at a number of venues across the county at various times and include:
